A check or money order for $250 must accompany your application for a foreign corporation in Pennsylvania.
Docketing Statement
The docketing statement, which is submitted with any new business filing in Pennsylvania, will contain information on where tax information should be sent, fiscal year end information, and an indication as to whether the foreign registration is for a for-profit or nonprofit.

The Application for Certificate of Authority is your Pennsylvania foreign registration document, similar to a domestic corporation's Articles of Incorporation.
What information should be provided in the Application for Certificate of Authority?
Your Application for Certificate of Authority should include the following information:
Name of corporation as it is registered with the domestic state
If the original name does not include a corporate designator (such as "Inc." or "Corp."), the name it will use in Pennsylvania that does include a corporate designator
If the above name is not available for use in Pennsylvania, the available fictitious name that the company will use in the state
The name of the domestic state
The principal office address in the domestic state
The Pennsylvania registered agent's name and address
A statement that the application is for a for-profit or a non-profit corporation
The signature of a corporate officer
In the case that the name of the corporation name as registered in the domestic state is unavailable for use in Pennsylvania and a fictitious name must be registered, a separate Fictitious Name Registration form must also be attached.
